Screw retained fixed Implant restorations have advantages over conventionally used Cement Retained Fixed Implant Restorations, which are most commonly used in dental implants. Cement Retained Restorations offer certain advantages such as:
- Management of improperly inclined implants,
- Passive fit,
- Absence of a visible screw access hole,
- Intact occlusal table and easier control of occlusion.
However one of the major dis-advantage of cement retained Fixed Implant Restoration is the difficulty in removing excess cement, which has been associated with the development of peri-implant mucositis and Peri-implantitis. Scientific studies have revealed the following Disadvantages of using Cement Retained- Teeth Implant Restorations:
- Excess cement remnants are left in almost all the cases
- Even the radiographs are unreliable way of determining excess cement.
- The different types of cement used also have been found of no help, as all the cement materials available are known to cause Peri-implantitis in Implants.
- Cannot be used in Inter-occlusal spaces less than 4 mm.
- Not Easy to remove and Maintain oral hygiene.
- Cemented Implant Restorations offer higher degree of Implant Technical Complications.
- Cement Retained Implant Restorations lead to greater rate of Fistula Formation and bacterial Infections.
- The procedures involved with cement retention for implant-supported crowns are not simple and should be carried out with great caution.
Screw-retained restorations have the advantage of more predictable results. They require a minimal amount of interocclusal space (4 mm) and are very easy to remove when Implant hygiene maintenance, repairs or corrections are required.
